BENEFITS BEFORE YOUR TRIP
PERIOD OF INSURANCE
If application for insurance is made online, by email, phone, or fax, coverage commences 24 hours after your premium payment via credit card is approved. If purchased by mail, coverage commences 24 hours after the postmark date. Coverage terminates on the outbound departure date specified in the application.
1. Cancellation Insurance: The Company shall reimburse you up to the sum insured for prepaid moneys forfeited to contracted travel suppliers for the entire cancellation of your reservation prior to departure provided the Company is notified immediately, in accordance within the Limitations, as soon as a cause for cancellation arises and provided further that the loss is due to one of the following hazards or events: (a) sickness, injury or death occurring to you, a member of your immediate family, a business partner or traveling companion; (b) transfer by your employer, subsequent to the booking date, requiring a move to a new permanent residence; (c) being selected for jury duty or a court procedure for which a subpoena is issued; (d) fire or other natural disaster that renders your principal residence uninhabitable; (e) being hijacked or quarantined; (f) terrorism in a country originally ticketed which leads the US Government to issue a general recommendation that Americans should not travel to or within that country for the period that would include your scheduled trip; and (g) death or hospitalization of your host at destination.
2. Missed Flight Insurance: The Company will indemnify you for the actual cost incurred of a one way economy airfare should: i) you miss the connection at the originally scheduled outbound departure point due to one of the events listed in Cancellation Insurance (a) or, ii) a delay of your connecting carrier (airline, bus, limousine, taxi, train) resulting from inclement weather conditions, mechanical failure or traffic accident, which must be substantiated by a police report, (subject to the connecting carrier being scheduled to arrive at the departure point not less than two hours prior to the originally scheduled departure time).
3. Upgrade Insurance: If one of the events listed in Cancellation Insurance (a) to (g) causes your traveling companion(s) to cancel his/her/their reservation but you elect to travel as planned, the Company will indemnify you for the upgrade charge paid to modify the room accommodation to reduced occupancy in the same hotel.

If purchased by mail, coverage commences 24 hours after the postmark date or on the outbound departure date of specified trip, whichever is later. Coverage terminates on the return date specified in the application, when you return to your residence in North America, or when the period ends for which premium was paid, whichever occurs first.
AUTOMATIC EXTENSION of PERIOD OF INSURANCE
Subject to the Conditions, Exclusions and General Conditions, your Period of Insurance will be extended without additional charge for a maximum of 72 hours, where due to circumstances beyond your control the journey is necessarily extended beyond the period stated in the application. In the event you are hospitalized, coverage will be automatically extended for the period of the hospital confinement and in addition, five days after release for both you and one insured traveling companion.

4. Travel
Delay: The Company will pay up to $100
a day to the Maximum Limit shown in the Schedule of Benefits if your trip
is delayed for more than 12 hours. This benefit is payable for only one
delay for all Insureds in order to cover reasonable, additional accommodation
and traveling expenses until travel becomes possible. Travel Delay must
be caused by: (a) carrier delay; or (b) lost or stolen passport, travel
documents or money; or (c) quarantine; or (d) natural disaster; or (e)
Injury or Sickness of the Insured or Traveling Companion.
5. Accidental Death, Dismemberment and Loss of Sight: If you sustain a covered injury during the policy period which results in any of the losses below within 120 days of a covered accident, the Company will pay the maximum sum for loss of life; loss of both hands; both feet; entire sight of both eyes; one hand and one foot; one hand or one foot and entire sight of one eye; or will pay one-half the maximum sum for loss of one hand; or one foot; or entire sight of one eye, as shown below:
(a) Common Carrier
Air Flights - pays benefits up to $15,000 for injuries sustained while
riding as a passenger, boarding or alighting from a flight of a multiple
engine transport type aircraft operated by a licensed airline maintaining
published schedules, or chartered airline or airport limousine or bus,
or surface vehicle substituted by the airline;
(b) Other Public
Conveyances - pays benefits up to 10,000 for injury sustained while riding
as a passenger, boarding or alighting from a conveyance provided by a common
carrier used primarily for passenger service such as taxis, buses, trains
and boats;
(c) 24 Hour
Accident - pays benefits up to $5,000 for injury sustained anywhere in
the world.
The three types of coverage are mutually exclusive, i.e. no more than one type of coverage is payable in the event of a covered accident.
6. Emergency Hospital/Medical Services: When, as a result of an injury or sickness which first occurs away from home while this Certificate is in force, you require the emergency services of a physician; the necessary emergency services of a chiropractor, chiropodist, podiatrist, osteopath, or physiotherapist; emergency hospital services; emergency x-ray clinic services; emergency ground ambulance service (air evacuation must be pre-approved by the Company); the necessary services of a registered graduate nurse; rental of a wheelchair, crutches, braces, hospital type bed or the cost of splints, trusses or other prosthetic appliances, medicine, drugs, blood, plasma or x-rays; the Company will pay up to $10,000 for reasonable and customary excess expenses incurred for emergency hospital/medical services listed above provided such services are prescribed in writing by the attending physician. Emergency Hospital services shall include all necessary services provided normally by a duly registered and licensed hospital excluding services of a nursing home, rest home or by other non-hospital institutions.
Pre-Existing Conditions: Pre-existing medical or related conditions are covered provided that you did not receive treatment or require the use of medication for said conditions during the 60 day period preceding the effective date of insurance. Notwithstanding the above, if a supervised condition not requiring treatment was controlled by the consistent use of medication and there was no alteration in the medication, dosage or usage during the 60 day period preceding the effective date of insurance, then such pre-existing condition will be covered. Pre-existing medical conditions are not covered if the condition was present prior to or on the date of departure and would have prevented an ordinarily prudent person from traveling.
When, as a result
of an injury or sickness, the attending physician prescribes and recommends
transportation to your original point of departure for further medical
treatment and to be accompanied by a trained medical attendant, the Company
shall pay the cost of one round-trip economy flight fare by the most direct
route for such trained medical attendant. Moreover, the Company will pay
the extra one-way economy flight fare costs should you be required by the
physician to travel bound to a stretcher. If the severity of your injury
or sickness warrants an emergency evacuation, the Company will pay up to
$25,000 for transportation from the place where you are injured or sick
to the nearest hospital where appropriate medical treatment may be obtained.
All transportation arrangements made for your evacuation must be by the
most direct and economical route possible. Evacuation must be recommended
by the attending physician in conjunction with a physician designated by
Worldwide Assistance Services (WA; see below) and approved in advance by
WA.

PRE-TRIP ASSISTANCE: Visa, passport, inoculation requirements; Cultural information; Temperature or weather conditions at destination; Embassy and Consular referrals; Foreign currency rates; Travel advisories.
EMERGENCY ASSISTANCE SERVICES: Medical referral and monitoring; Medical evacuation or repatriation; Repatriation of remains; Traveling companion or dependent children assistance; Prescription assistance; Urgent message relay; Travel arrangements; Advance of cash; Legal assistance/bail; Telephone interpretation. You must call the appropriate telephone number listed above immediately in the event of a medical emergency. Failure to contact the assistance provider will result in the compensation being declined or substantially reduced unless the Insured is unconscious or physically unable to contact Worldwide Assistance.
In all cases, the medical professional, medical facility and/or attorney suggested by WA or providing direct services to the eligible person are not employees or agents of WA, and the final selection of the medical professional, medical facility, or legal counsel is your choice alone. WA assumes no responsibility for any medical advice or legal counsel given by the medical professional and/or attorney, nor shall WA be liable for the negligence or other wrongful acts or omissions of any of the legal and/or health care professionals providing direct services. You shall have no recourse against WA by reason of its suggestion of or contract with a medical professional and/or attorney.
7. Emergency Dental Benefit: If, as a result of an accidental blow to the face, emergency dental treatment to natural teeth is required, the Company shall pay for expenses incurred not to exceed $1,000. The Company shall also pay up to $300 for other emergency dental treatment for the relief of pain (excluding root canal) at the destination.
8. Out-of-Pocket Expenses: In the event a covered injury or sickness causes your delay in returning to the point of departure beyond the return date, the Company will pay for reasonable out-of-pocket expenses you incur for meals, commercial lodging and laundry expenses up to $150 per day to a maximum of $3,000. Original receipts must be submitted to the Company.
9. Return Flight Insurance: If one of the events listed in Cancellation Insurance (a) to (g) forces you to miss the contracted return flight, this insurance will reimburse you up to the cost of similar one-way economy class transportation from the contracted destination to the original departure point. Written justification must be obtained from the attending physician.
10. Unused Travel Arrangements: If you must return to the original departure point prior to the scheduled return date due to one of the events listed in Cancellation Insurance (a) to (g), the Company will reimburse you up to the sum insured for unused travel arrangements which will not be refunded by contracted travel suppliers.
11. Trip Interruption: If, after the outbound departure, you are obliged to leave the trip due to personal sickness or injury, the Company will pay for the cost of one-way economy class transportation to rejoin the ongoing trip.
12. Return of Vehicle Benefit: If, as the result of a medical emergency, you become totally disabled, the Company will pay the actual expense incurred for the return of the vehicle to your regular place of residence, or, if a rented vehicle from a Commercial Car Rental Agency, to the nearest location of such car rental agency, but not to exceed $1,000.
13. Family Transportation Expense: Should you be hospitalized due to a medical emergency and the attending physician advises the necessary attendance of one immediate family member, the Company shall pay up to $1,000 for the reasonable cost of transportation by economy class (such immediate family member shall not be deemed as an insured person under this policy).
14. Repatriation Expense: In the event of your covered death (occurring after the departure date), the Company will pay either the cost of the actual expense incurred for preparation plus transport via standard body container to the outbound point of departure (up to $5,000) or for the reasonable costs for burial or cremation abroad, not to exceed $1,500.
EXCLUSIONS
Not covered are expenses or losses incurred directly or indirectly as a result of: injuries occurring before coverage is in force; sickness which reveals itself by symptoms before coverage is in force; injury or sickness for which hospital or medical benefits are provided under any other insurance plan except for the excess not covered under such other insurance; maintenance therapy for existing medical conditions, or hospital/medical expenses incurred on an elective non-emergency basis; suicide or self destruction, intentionally self- inflicted injuries or any attempt thereat, while sane or insane; declared or undeclared war, civil war, riot, insurrection, invasion, or any act thereof; an illegal act by the Insured or beneficiary; participation in armed forces training exercises or maneuvers; participation in sporting events of a professional nature for remuneration, motor or racing competition, mountaineering, parachuting, skydiving, piloting an aircraft; or underwater activities of a professional nature; pregnancy, miscarriage, childbirth or complication within eight weeks of the expected date of birth; hospital and medical expenses for any person not named as an insured person; a journey undertaken for the purpose of securing or with the intent of receiving medical attention, hospital or dental services; expenses incurred while under the influence of drugs, medication, alcohol or other intoxicants; mental health disorders unless hospitalized.
LIMITATIONS
Cancellation insurance settlements are limited to the amount of penalty involved at the time cancellation is first recommended by the physician. No benefit will be payable if you do not cancel the insured trip within 48 hours of the cause for cancellation arising. In addition, no benefit will be payable if you receive substitute transportation to the scheduled return destination or to rejoin the trip. Expenses incurred for necessary emergency medical services are covered only if you return to your permanent residence as soon as medically possible. If injury or sickness delays your return to the original departure point for more than 10 days beyond the original return date, no benefits will be paid unless you are confined to a hospital. Insurance premiums are non-refundable.
15. Baggage/Personal
Effects: This Certificate insures against
all risk of loss or damage to property sustained while in transit or while
in any hotel or other building, not owned by the Insured, en route during
any journey anywhere in the world on water, land or in the air within the
Certificate period, except as otherwise provided herein, up to $1,000 or
the maximum selected but not exceeding $300.00 for any one item. Baggage
insurance does not cover any loss or damage: a) caused by wear and tear;
b) to property specifically otherwise insured (declaration of value on
checked baggage not to be considered other insurance); c) sporting equipment
where such loss or damage is due to the use thereof. This insurance covers
only in excess of any homeowners' or other property insurance. Failure
to obtain a proof of loss report (from police, airline, hotel or other
authority where loss occurs) may result in denial of benefits. A
$50 deductible will be applied to every loss.
Baggage Delay:We
will pay up to $100 for the cost of reasonable, additional clothing and
personal articles you purchase if your baggage is delayed for more than
24 hours. This does not apply if baggage is delayed after you arrive at
your return destination.
DEFINITIONS
"Baggage" means personal effects and property owned by and for your personal use, adornment or amusement while in your possession on the insured trip. However, the term "baggage" does not include animals, automobiles, automobile equipment or contents, motorcycles, bicycles, boats, motors, trailers, mobile homes or other conveyances or their appurtenances (other than wheelchairs), household furniture, contact lenses, safety deposit keys, credit cards, documents, statuary, paintings, china, glass objects and objects of art or antiques, collectors items, artificial teeth and limbs, securities, money, tickets, documents, any property pertaining to business, profession or occupation, fragile or brittle articles, perishables, cigarettes and alcohol.
"Death" wherever used in connection with Benefit #1 means death resulting from "Injury" or "Sickness" as defined herein.
"Economy Flight Fare" means the lowest one-way single seat fare of any ATC or IATA Air Carrier.
"Eligible Person" means a person who is a permanent resident of the USA who is not over 75 years of age.
"Hospital" means a duly licensed hospital with facilities to accommodate inpatient care with a registered nurse on a full time duty basis, a laboratory and an operating room where surgical operations are performed by a qualified surgeon. Not included are convalescent homes, rest homes, nursing homes, homes for the aged, drug & alcohol treatment centers, health spas or clinics.
"Host at Destination" means the person with whom you are sharing pre-arranged overnight accommodation in his/her principal place of residence.
"Immediate Family Member" means your legal or common-law spouse, parent, stepparent, grandparent, natural or adopted child, grandchild, siblings, stepbrother or stepsister, siblings-in-law, parents-in-law, children-in-law, uncle, or aunt.
"Injury" means bodily injury caused by an accident occurring while the Certificate is in force, resulting directly and independently of all other causes in loss covered by this Certificate.
"Insured" means a person who is eligible for the insurance applied for and who has paid the full premium required in advance.
"Loss" as referred to in Benefit #1, means the amount of money paid to or due to contracted suppliers for travel arrangements (air fares, land arrangements, hotel deposits, cruises, etc.)
"Physician" means any person, other than the insured or any member of his or her immediate family, who is a practitioner of the healing arts whose legal and professional standing within his/her jurisdiction is equivalent to a doctor of medicine (M.D.) duly licensed to practice in the USA.
"Pre-existing medical condition" means an injury or sickness of an Insured person or a Family Member which manifests itself during the 60 day period immediately preceding and including the Effective Date, unless the condition is controlled through taking prescription drugs or medication and remains controlled throughout the 60 day period. A sickness has manifested itself when: (a) medical care or treatment has been given; or (b) symptoms exist which would cause a reasonably prudent person to seek medical advice, diagnosis, care or treatment.
"Sickness" means sickness or disease contracted and commencing while the Certificate is in force.
"Traveling Companion" means a person named on the Enrollment Form who will accompany you on the entire trip (maximum of 3 persons).
"You" and "Your" means the insured person.
This individual certificate is furnished in accordance with and is subject to the provisions, conditions, and limitations of the master policy, which constitutes the entire contract. It replaces any and all certificates which may have been issued to you previously under the Policy. No coverage will be in force if (a) the Trip Departure Date and Trip Completion Date do not reflect your intent to start and end your trip on those dates; (b) your tickets do not contain specific travel dates (open tickets); or (c) promotional coupons or tickets issued on non-Airline Reporting Corporation approved ticket stock other than Frequent Flyer Awards are used.
Notice to Residents
of California: This plan contains disability insurance benefits or health
insurance benefits, or both, that only apply during the covered trip. You
may have coverage from other sources that already provides you with these
benefits. You should review your existing policies. If you have any questions
about your current coverage, call your insurer or health plan.
HOW TO PRESENT YOUR CLAIM
In the event of trip cancellation, notify your travel supplier(s) without delay. Retain all receipts and unused transportation tickets. If your belongings are damaged, lost, or stolen you must immediately notify the authorities (transportation carrier, hotel manager, tour/cruise representative or local police) and obtain a written report. All claims are to be made in writing within 30 days to the Administrator:
